Pharmacotherapeutic group

ATC code- A02AB10. Antacids, aluminum compounds.

Almagel - a drug that reduces acidity gastric juice... It is a balanced combination of aluminum and magnesium hydroxides, which are insoluble and hardly available to the body. Almagel has a local effect on the gastric mucosa, protects it from the irritating effect of acids and other harmful substances and food, reduces the activity of pepsin. The drug reduces the acidity in the esophagus. It has an anti-inflammatory and astringent effect on the mucous membrane of the esophagus, stomach and duodenum. Excipient sorbitol has a weak carminative and moderate choleretic effect, as well as a moderate laxative effect.

Indications for use

Symptomatic treatment of diseases digestive tract accompanied by increased acidity of gastric juice: esophagitis, hernia esophageal opening diaphragm, gastroesophageal reflux disease (GERD), acute and chronic gastritis, gastroluodenitis, peptic ulcer stomach and duodenum, post-resection gastritis and gastroanastomositis in the presence of gastroesophageal reflux (heartburn). Prophylactically to reduce the irritant and ulcerogenic effects of some medicines on the mucous membrane of the esophagus, stomach and duodenum (for example, corticosteroids, nonsteroidal anti-inflammatory drugs).

Contraindications

The use of Ashiagel is not recommended in case of hypersensitivity to the components of the drug, habitual constipation, Alzheimer's disease, with severe pain in the abdomen unspecified genesis, if you suspect acute appendicitis, availability ulcerative colitis, colostomy or ileostomy ‚with chronic diarrhea, hemorrhoids, severe renal failure, hypophosphatemia, during lactation, children under 10 years of age.

Precautions for use

Keep the following in mind when using this medication:

In patients with severe constipation; with pain in the stomach of unknown origin and with suspicion of acute appendicitis; in the presence of ulcerative colitis, diverticulosis, colostomy or ileostomy (increased risk of developing disorders of water and electrolyte balance); chronic diarrhea; exacerbation of hemorrhoids; when the acid-base balance in the body changes and especially in the presence of metabolic alkalosis; with cirrhosis of the liver; with decompensated congestive heart failure; with toxicosis of pregnant women; with renal failure (the risk of developing hypermagnesemia and aluminum intoxication).

Patients should see a doctor if they lose weight, have difficulty swallowing, or constant feeling discomfort in the abdomen, digestive disorders that appeared for the first time; or when changing the course of existing digestive disorders.

Aluminum hydroxide can lead to constipation and magnesium hydroxide can lead to intestinal hypokinesia. Application of this product in high doses may cause or worsen bowel obstruction and intestinal obstruction, especially in patients with an increased risk of such complications, for example, in patients with renal failure or in elderly patients.

The use of antacids containing aluminum in elderly patients should be limited.

With prolonged use of the drug, it is necessary to ensure that a sufficient amount of phosphorus is supplied to the body, since aluminum hydroxide binds to phosphates and reduces their absorption from the digestive tract. The excretion of calcium in the urine increases, which can lead to disturbances in calcium-phosphate balance and create conditions for the development of osteomalacia (symptoms are complaints of weakness and pain in the bones).

Aluminum hydroxide can be hazardous when used in hemodialysis patients with porphyria.

Since aluminum is concentrated in nervous tissue, the use of aluminum-containing antacids in elderly patients and patients with Alzheimer's disease should be strictly limited. When taken by elderly patients, the course may worsen existing diseases bones and joints.

With prolonged use of the drug (more than 14 days), regular medical supervision and determination of the magnesium content in the blood are required in the treatment of patients with renal failure.

Interaction with other medicinal products and other forms of interaction

Please inform your doctor or pharmacist if you are taking or have recently taken other medicines, even if they have not been prescribed for you.

With simultaneous treatment, other drugs should be taken 1-2 hours before or after taking Almagel.

Almagel reduces the acidity of gastric juice, and this can affect the action a large number medicines while taking.

Almagel reduces the absorption of digoxin ‚indomethacin, salicylates, chlorpromazine, phenytoin, reserpine, H2-blockers (cimetidine, ranitidine, famotidine), lansoprazole, β-blockers (for example, atenolol, metoprolol, propranoloniolone, chloroquine ciprofloxacin, norfloxacin, ofloxacin, enoxacin, grepafloxacin), azithromycin, cefpodoxime, pivampicilin, rifampicin, indirect anticoagulants, barbiturates, fexofenadine, sodium fluoride, dipyridamole, zalcitabine, bile acids (chenodeoxycholic and ursodeoxycholic acids), penicillamine, lansoprozole, digitalis glycosides, iron salts, lithium preparations, quinidine, mexaziletine drugs, antibiotics anti-tuberculosis drugs (ethambutol, isoniazid (for oral use), chloroquine, gshococorticoid drugs (known to interact with prednisolone and dexamethasone), kayekselat and ketoconazole.

With the simultaneous administration of enteric drugs, the increased alkalinity of gastric juice can lead to accelerated destruction of their membrane and cause irritation of the stomach and duodenum.

Almagel can affect the results of some laboratory and functional studies and tests: it reduces the level of gastric secretion and thus alters the results of a functional study of its acidity; alters the results of tests using technetium (Tc99) ‚for example, bone scintigraphy and some tests for examining the esophagus, increases serum phosphorus, serum and urine pH values.

The use of the drug and the intake of food and drinks

Application during pregnancy and lactation

Before taking any medicine, ask your doctor or pharmacist for advice.

There is no clinical data on the use of Almagel by pregnant women. The drug is not recommended during pregnancy, but if the drug is needed, then the duration of treatment should not exceed 5-6 days, and the drug should be taken under medical supervision. It is not recommended to prescribe for toxicosis of pregnant women.

There is no data on the intake of active substances of the drug in difficult milk. Almagel can be used during breastfeeding only after a careful assessment of the ratio of benefits to the mother and the potential risk to the newborn.

Impact on management ability vehicles and work with equipment

Almagel does not affect the ability to drive a car and work with equipment. The ethanol included in the preparation is contained in quantities that are not able to affect the ability to drive and use equipment while taking the recommended daily doses.

Information on excipients

The drug contains sorbitol, which allows diabetics to take it, but is not suitable for the treatment of patients with congenital fructose intolerance, as it can cause stomach irritation and diarrhea.

The suspension contains parahydroxybenzoates, which can cause an allergic reaction (possibly of a delayed type) and, very rarely, bronchospasm.

The medicinal product contains 2.5 vol% ethyl alcohol (ethanol), which corresponds to the content of 98.1 mg ethanol in 5 ml and 196.2 mg ethanol in 10 ml of the medicinal product. Due to the presence of ethyl alcohol in Almagel, this drug should be used with caution in pregnant women, lactating women, children and patients with liver disease, epilepsy and alcohol dependence.

pharmachologic effect

Antacid preparation, which is a balanced combination of algeldrate (aluminum hydroxide) and magnesium hydroxide. Neutralizes free hydrochloric acid in the stomach, reduces the activity of pepsin, which leads to a decrease in the digestive activity of gastric juice.

It has an enveloping, adsorbing effect. Protects the gastric mucosa by stimulating the synthesis of prostaglandins (cytoprotective effect). Protects the mucous membrane from inflammatory and erosive-hemorrhagic lesions as a result of the use of irritating and ulcerogenic agents such as ethanol, NSAIDs (for example, indomethacin, diclofenac), corticosteroids.

The therapeutic effect after taking the drug occurs in 3-5 minutes. The duration of action depends on the rate of gastric emptying. When taken on an empty stomach, the effect lasts up to 60 minutes. When taken one hour after a meal, the antacid effect can last up to 3 hours. Does not cause secondary hypersecretion of gastric juice.

Pharmacokinetics

Algeldrat

A small amount of the drug is absorbed, which practically does not change the concentration of aluminum salts in the blood. It is excreted through the intestines.

Magnesium hydroxide

Magnesium ions are absorbed in insignificant amounts (about 10% of the dose taken) and do not change the concentration of magnesium in the blood. It is usually distributed locally. Magnesium hydroxide is excreted through the intestines.

With prolonged use in high doses, formation, development of severe constipation, slight drowsiness, hypermagnesemia is possible. Signs of metabolic alkalosis can also be observed: changes in mood or mental activity, numbness or muscle pain, irritability and fatigue, slow breathing, unpleasant taste sensations.

Treatment: it is necessary to immediately take measures for the rapid elimination of the drug from the body - gastric lavage, stimulation of vomiting, intake of activated carbon.

Drug interactions

It can adsorb some drugs, thus reducing their absorption. Therefore, with the simultaneous use of other drugs, it is necessary to observe an interval of 1-2 hours between taking Almagel and other drugs.

Almagel changes the pH of gastric juice to the alkaline side, which can affect the effect of a significant number of drugs when used simultaneously.

Almagel reduces the effect of blockers of histamine H 2 -receptors (, ranitidine, famotidine), cardiac glycosides, iron salts, lithium preparations, quinidine, mexiletine, phenothiazine drugs, tetracycline antibiotics, ciprofloxacin, isoniazid and ketoconazole.

With the simultaneous appointment of enteric forms of drugs, it should be remembered that an increase in the pH of gastric juice caused by the intake of Almagel can lead to accelerated destruction of the enteric membrane and thereby cause irritation of the mucous membrane of the stomach and duodenum.

Almagel can affect the results of some laboratory and functional studies and tests: reduces the level of gastric secretion when determining the acidity of gastric juice; alters the results of tests using technetium (99m Tc), such as bone scintigraphy and some tests for examining the esophagus; increases the concentration of phosphorus in the blood serum, changes the pH values of blood serum and urine.

special instructions

It is not recommended to use the drug in patients with severe constipation, with pain in the stomach of unknown origin and suspicion of ulcerative colitis, diverticulosis, colostomy or ileostomy, chronic diarrhea, acute hemorrhoids, changes in the acid-base balance in the body, as well as the presence of metabolic alkalosis, severe heart failure, toxicosis of pregnant women, impaired renal function ((QC<30 мл/мин) из-за опасности развития гипермагниемии и алюминиевой интоксикации).

The drug does not contain sugar, which allows patients with diabetes mellitus to take it.

The drug contains sorbitol, therefore it is contraindicated in congenital fructose intolerance.

Influence on the ability to drive vehicles and use mechanisms

Almagel does not affect the ability to drive and operate machinery. When taken in the recommended daily dose, the ethanol contained in the preparation does not affect the ability to drive and operate machinery.

In the article we will consider the instruction for children to "Almagel".

It is an antacid drug for the treatment of diseases of the digestive system. Its active components neutralize hydrochloric acid, which is included in gastric juice. The composition of this drug includes aluminum hydroxide, which is the basis for the manufacture of antacid medicines.

Not everyone knows that Almagel is suitable for children.

Kinds

The release forms of this medication are suspension and tablets. The medication is sold in three versions:

  • The usual "Almagel" (it is produced in the form of liquid and tablets). This version of Almagel contains the main constituents such as aluminum hydroxide along with magnesium.
  • A drug with the additional designation "A". This type contains benzocaine, which provides pain relief.
  • Variety "Almagel Neo", in addition to the main components, may contain simethicone, which removes excess gases.

Is it true that Almagel is allowed for children? This is a common question.

How do they differ?

All types of the drug are sold in boxes that differ in color. For example, the usual "Almagel" is sold in green packaging, "Neo" is sold in red, and with the designation "A" - in yellow. The name "Almagel T" is used for the pills. The drug with a special additive "Neo" is produced in sachets of 10 milliliters, it contains orange flavor in its composition.

How does Almagel work for children?

The action of the medication

The presented medicine produces an enveloping and antacid effect. All its types contain the same basic active active ingredient. The mechanism of action is as follows: the drug neutralizes hydrochloric acid, protecting the gastric mucosa, which effectively prevents the formation of ulcers.

At the same time, components that can negatively affect the mucous membrane of the digestive system are neutralized. The composition of this medicine envelops the stomach, creating a protective barrier. The active components of the drug are distributed evenly, thereby achieving a long-lasting effect.

Sodium hydroxide eliminates and prevents constipation. Sorbitol, which is part of the composition, increases the secretion of bile. Both components contribute to the rapid regulation of the stool. The protective layer protects the stomach from the accumulation of gases, a feeling of heaviness and excessive production of hydrochloric acid.

The presented remedy eliminates pain in the digestive organs. It is also possible to use "Almagel" for heartburn. The effect of the drug begins three to five minutes after ingestion. The duration of the effect is from one to two hours.

"Almagel A" has a longer and stronger analgesic effect. It can be taken in case of poisoning. The medicine with the postscript "Neo" eliminates and prevents gas formation. Gas bubbles are absorbed into the intestines, and then completely removed from the body. This medication can cause some side effects and has a number of contraindications. For pregnant women, it is prescribed only in short courses.

In what cases is Almagel prescribed for children?

Indications for use for children

This drug is a non-absorbable agent and does not cause any metabolic disturbances. "Almagel" in the presence of heartburn is used even during pregnancy. This medication does not irritate the genitourinary system, even with prolonged use. The indications for use in children are as follows:

  • Against the background of intestinal diseases.
  • In the presence of flatulence.
  • With enteritis, colitis and gastritis.
  • Against the background of reflux esophagitis.
  • Against the background of the consequences of an improper diet.

It is allowed to take "Almagel" for children as an aid in the treatment of diabetes, but only on the recommendation of a doctor. This drug can be given together with other medicines or immediately after taking non-steroidal drugs and glucocorticosteroids.

After using the suspension, it is forbidden to give the child any drink for half an hour. The use of "Almagel Neo" and other drugs must be distributed over time. The suspension itself is taken one hour after a meal. The use of the drug for a long time must necessarily be accompanied by the inclusion in the diet of foods that are rich in high phosphorus content.

Children who are between the ages of ten and fifteen years, take the suspension one measuring spoon (that is, 5 milliliters of medicine) four times. The last dose is given to the child to drink in the evening, just before bedtime. In the event that the baby's condition is severe, and the symptoms are excessively pronounced, then a one-time intake can be increased to two measuring spoons, which will correspond to 10 milliliters of medicine. The maximum allowable rate of "Almagel", which is allowed to be taken by children aged ten to fifteen years during the day, is six measuring spoons. The duration of the course of therapy should not exceed four weeks.

Side effects and effects on the child's body

This drug works differently on the child's body. Some kids easily tolerate treatment with Almagel, while others may experience some side effects while using it. Thus, as a result of taking this medication, children can sometimes experience various side effects, but most often this happens due to an overdose and improper adherence to the amount of the drug prescribed by the doctor. So, the drug "Almagel" in green and yellow packaging can cause undesirable reactions in children in the form of nausea, vomiting, stomach cramps, drowsiness, changes in the action of taste buds and constipation.

It is also worth noting that prolonged use of this medication leads to phosphorus deficiency and bone destruction. In the event that the drug is used for renal failure, this drug can provoke swelling of the hands and feet along with the appearance of dementia and an increase in the concentration of magnesium in the blood. The form of "Almagel" called "Neo" sometimes provokes allergic reactions in children, as well as a violation of the stool.

Special instructions for use

It should be borne in mind that immediately before taking the medicinal suspension should be shaken well to a homogeneous composition. It is not recommended to use "Almagel" in excess of the amount established by the doctor, otherwise the child may experience undesirable side reactions. And the onset of an overdose can cause an increase in the amount of magnesium and aluminum in the blood.

Phosphorus deficiency due to overdose can cause osteoporosis, and if the child does not have enough calcium, then frequent fractures are possible. In the kidneys, against the background of excessive intake of this medication, salt deposits appear, and at the same time the normal activity of various organs is disrupted. Sometimes an overdose can cause encephalopathy. Children with kidney failure may have thirst along with low blood pressure.

The presented drug is a slightly toxic medication, thanks to which it is allowed in small quantities even for pregnant women. Thus, this remedy, provided that the dosage is observed, does not pose any threat to the health of the child and prevents relapses of diseases of the digestive system.

Almagel is rightfully considered one of the popular antacid drugs, the indications for the use of which are not limited only to relieve heartburn. It is widely used for ulcers, duodenitis, esophagitis and other acid-related diseases. Possesses good enveloping and absorbent properties.

The drug Almagel also has the spelling "Almagel". This confusion is due to the fact that the name of the original is written in Latin letters "Almagel".

Composition and form of release

Almagel is a combined agent produced in the form of a gel that acts as a protective film on the gastric mucosa. Stimulates the outflow of bile from the stomach and has a laxative effect on the body. Unlike other drugs, it does not disturb the electrolyte balance in the gastrointestinal tract.

The suspension contains various components that give the drug certain additional properties.

  1. Suspension Almagel (contains only the main components - gel of aluminum and magnesium hydroxide);
  2. Suspension Almagel A (together with the main components contains anesthetic benzocaine);
  3. Suspension Almagel Neo (together with the main components contains the substance simethicone, which removes gases).

White suspension intended for oral administration. Has a specific lemon scent. During long-term storage, a transparent layer of liquid forms on the surface. Homogeneity is restored with vigorous agitation of the solution.

pharmachologic effect

Almagel is a drug that is a balanced combination of algeldrate (aluminum hydroxide) and magnesium hydroxide. It neutralizes free hydrochloric acid in the stomach, reduces the activity of pepsin, which leads to a decrease in the digestive activity of gastric juice. It has an enveloping, adsorbing effect.

In addition, Almagel is able to provide a local anesthetic effect, which helps to overcome the main symptom of gastrointestinal diseases - pain. The medicinal substance is also capable of taking part in the production of bile (accelerates and improves the excretion of bile). Almagel can be attributed to the group of laxative medicines due to the content of the D-sorbitol component.

The drug forms a protective film on the walls of the stomach, preventing the formation of carbon dioxide, which is the cause of:

  • feelings of heaviness in the epigastrium,
  • increased gassing,
  • secondary hypersecretion.

The drug has no mutagenic, embryotoxic and teratogenic effects.

Pharmacokinetics:

  1. Almagel is a non-absorbable drug. Subject to the correct dosage regimen and the duration of treatment, it is practically not resorbed in the gastrointestinal tract and has a long-term uniform effect without disturbing the electrolyte balance and without creating the risk of alkalosis or other metabolic disorders.
  2. Does not irritate the urinary system and, with prolonged use, does not cause alkalosis and the formation of calculi in the urinary tract.
