I. Organization of high-tech medical care
1. This procedure establishes the rules for organizing high-tech medical care using a specialized information system in medical organizations providing high-tech medical care.
2. High-tech medical care, which is part of specialized medical care, includes the use of new complex and (or) unique methods of treatment, as well as resource-intensive treatment methods with scientifically proven efficacy, including cellular technologies, robotic equipment, information technologies and gene methods Engineering developed based on the achievements of medical science and related industries of science and technology 1.
3. High-tech medical care is provided in accordance with the procedures for providing medical care and based on medical care standards.
4. High-tech medical care is in the following conditions:
4.1. In day hospital (under conditions providing for medical observation and treatment during daytime, but not requiring round-the-clock medical observation and treatment);
4.2. Stationary (in conditions providing round-the-clock medical observation and treatment).
5. High-tech medical care is provided in accordance with the list of types of high-tech medical care, established by the program of state guarantees of free provision of medical care citizens 2, which includes:
5.1. The list of high-tech medical care types included in the basic health insurance program, financial support, which is carried out at the expense of subventions from the budget of the Federal Fund for Compulsory Medical Insurance Bids of Territorial Mandatory Medical Insurance Funds;
5.2. 6. High-tech medical care for the list of species included in the basic compulsory health insurance program is provided by medical organizations included in the register of medical organizations operating in the field of compulsory health insurance.
7. High-tech medical care for the list of species not included in the basic health insurance program is:
7.1. Federal State Institutions, the list of which is approved by the Ministry of Health of the Russian Federation in accordance with Part 6 of Article 34 of the Federal Law of November 21, 2011 N 323-FZ "On the Fundamentals of the Protection of Citizens in the Russian Federation";
7.2. Medical organizations, the list of which is approved by the authorized body of the executive authority of the constituent entity of the Russian Federation (hereinafter referred to as a list of medical organizations) in accordance with Part 7 of Article 34 of the Federal Law of November 21, 2011 N 323-FZ "On the basis of the health of citizens' health in the Russian Federation."
8. The authorized executive authority of the constituent entity of the Russian Federation submits a list of medical organizations in the Ministry of Health of the Russian Federation for December 20, preceding the reporting.
9. The federal fund of compulsory health insurance on the basis of information of territorial compulsory medical insurance funds is a list of medical organizations included in the register of medical organizations operating in the field of compulsory health insurance and providing high-tech medical care providing high-tech medical care. Included in the basic compulsory medical insurance program, up to December 10 of the year preceding the reporting.
10. The Ministry of Health of the Russian Federation forms a list of medical organizations in a specialized information system providing high-tech medical care in accordance with paragraphs 4-6 of this Procedure, up to December 30 of the year preceding the reporting.
II. Direction for high-tech medical care
11. Medical testimony for the provision of high-tech medical care determines the attending physician of a medical organization in which the patient undergoes diagnosis and treatment within the provision of primary specialized health care and (or) specialized medical care, taking into account the right to choose a medical organization 3.
12. Medical indications for the provision of high-tech medical care is the presence of a patient with a disease and (or) states requiring the use of high-tech medical care in accordance with the list of types of high-tech medical care.
13. In the presence of medical testimony to provide high-tech medical care attending a medical organization, in which the patient undergoes diagnosis and treatment in the provision of primary specialized health care and (or specialized medical care (hereinafter - the guide medical organization) issues a direction to hospitalization To provide high-tech medical care on the form of a guide medical organization, which should be written in hand or in print, certified by the personal signature of the attending physician, personal signature of the head of the medical organization (authorized person), the seal of the attending physician, the seal of the guide medical organization and contain the following intelligence:
13.1. Surname, first name, patronymic (if any) patient, date of birth, registration address at the place of residence (stay);
13.2. The number of compulsory health insurance and the name of the insurance medical organization (if available);
13.3. Insurance certificate of compulsory pension insurance (if available);
13.4. The diagnosis code of the main disease according to the ICD-10 4;
13.5. Profile, name of high-tech medical care in accordance with the list of types of high-tech medical care shown by the patient;
13.6. The name of the medical organization in which the patient is sent to provide high-tech medical care;
13.7. Surname, name, patronymic (if any) and the position of the attending physician, contact phone (if available), email address (if available).
14. The following patient documents are attached to the hospitalization for high-tech medical care:
14.1. Extract from medical records certified by the personal signature of the attending physician, personal signature of the head (authorized person) of a guiding medical organization containing a diagnosis of the disease (state), the diagnosis code for the ICD-10, information on the patient's health status, the results of laboratory, instrumental and other types of research confirming the diagnosed diagnosis and the need to provide high-tech medical care.
14.2. Copies of the following patient documents:
a) a document certifying the patient's identity (the main document certifying the identity of a citizen of the Russian Federation in the Russian Federation is a passport 5;
a document certifying the person of a person who worked on hiring, busy or working in any capacity aboard the sea vessel (with the exception of a warship), the sea vessel of the fishing fleet, as well as the vessel of the mixed (river - the sea) of swimming, used for commercial navigation purposes, is Sailor ID 6;

14.3. Consent to the processing of personal data of the patient and (or) of his legal representative.
15. The Guide Medical Organization presents a set of documents provided for in paragraphs 13 and 14 of this Procedure, within three working days, including through a specialized information system, postal and (or) electronic communications:
15.1. The medical organization included in the register of medical organizations operating in the field of compulsory health insurance, in the event of a high-tech medical care included in the basic health insurance program (hereinafter - the host medical organization);
15.2. The executive authority of the constituent entity of the Russian Federation in the field of health (hereinafter - the OUS) in the event of a high-tech medical care, not included in the basic compulsory medical insurance program.
16. The patient (his legal representative) is entitled to independently submit a decorated set of documents in the OUN (in the case of high-tech medical care, not included in the basic health insurance program), or to a host medical organization (in the case of high-tech medical care included in the basic Mandatory Medical Insurance Program).
17. In the direction of the patient in the receiving medical organization, the registration of high-tech medical care on the patient for the provision of high-tech medical care (hereinafter referred to as the provision of VMM) with the use of a specialized information system provides a host medical organization with the attachment of a set of documents provided for in paragraphs 13 and 14 of this Procedure.
18. In the direction of the patient for the provision of high-tech medical care, not included in the basic program of compulsory medical insurance, the execution of the Talon for the provision of a VMM with the use of a specialized information system provides an OUC with the attachment of the set of documents provided for by 13 and 14 of this Procedure and the conclusion of the commission of the executive authority of the subject The Russian Federation in the field of health care on the selection of patients to provide high-tech medical care (hereinafter referred to as the OUN Commission).
18.1. The term for the preparation of the decision of the Commission for the confirmation of the presence (on the absence) of medical testimony to send a patient to the receiving medical organization to provide high-tech medical care should not exceed ten working days from the date of receipt of a set of documents provided for in paragraphs 13 and 14 of this Procedure.
18.2. The decision of the OUZ Commission is issued by the Protocol containing the following information:
18.2.1. The basis of the creation of the OUZ Commission (details of the regulatory legal act);
18.2.2. The composition of the Commission of the OUS;
18.2.3. Patient data in accordance with the identity document (surname, name, patronymic, date of birth, data on the place of residence, (stay));
18.2.4. Diagnosis of the disease (state);
18.2.5. Conclusion of the OUZ Commission containing the following information:
a) on confirmation of medical testimony to send a patient to a medical organization for the provision of high-tech medical care, a diagnosis of the disease (state), the diagnosis code for the ICD-10, the code of the type of high-tech medical care in accordance with the list of types of high-tech medical care, the name of the medical organization in which the patient is sent to the provision of high-tech medical care;
b) on the lack of medical indications for the direction of the patient to the medical organization to provide high-tech medical care and recommendations for further medical supervision and (or) treatment of the patient in the profile of its disease;
c) on the need for an additional survey (indicating the necessary amount of additional examination), diagnosis of the disease (state), the diagnosis code for the ICD-10, the name of the medical organization, to which it is recommended to send a patient for an additional survey.
18.3. The OUZ Commission's decision is made in two copies, one copy is subject to storage for 10 years in the UUC.
18.4. An extract from the protocol of the decision of the Uous Commission is sent to the Guide Medical Organization, including by postal and (or) electronic communications, and is also issued to the patient (his legal representative) on a written statement or is sent to the patient (his legal representative) by means of postal and ( or) electronic communications.
19. The basis for the hospitalization of the patient to the host medical organization and medical organizations provided for in paragraph 5 of this Procedure (hereinafter referred to as medical organizations providing high-tech medical care) is the decision of the Medical Organization's Medical Commission in which the patient is sent to the selection of patients to provide high-tech medical care. , (hereinafter - the Commission of a Medical Organization, providing high-tech medical care).
19.1. The Commission of a Medical Organization, providing high-tech medical care, is formed by the head of a medical organization providing high-tech medical care 10.

19.2. 19.3. The decision of the Commission of a Medical Organization, which provides high-tech medical care is issued by the protocol containing the following information:

2) the composition of the Commission of a Medical Organization providing high-tech medical care;
3) patient data in accordance with the identity document (surname, name, patronymic, date of birth, data on the place of residence (stay));
4) Diagnosis of the disease (state);
5) Conclusion of the Commission of a Medical Organization providing high-tech medical care containing the following information:
a) about the presence of medical testimony and the planned date of the patient's hospitalization into a medical organization providing high-tech medical care, a diagnosis of the disease (state), the diagnosis code for the ICD-10, the code of the type of high-tech medical care in accordance with the list of types of high-tech medical care;
6) on the absence of medical testimony to hospitalize the patient to a medical organization providing high-tech medical assistance with recommendations for further medical supervision and (or) treatment of the patient in the profile of its disease;
c) on the need for an additional examination (indicating the necessary amount of additional examination), diagnosis of the disease (state), the diagnosis code for the ICD-10, indicating a medical organization, which is recommended to send a patient for an additional examination;
d) on the presence of medical testimony to send a patient to a medical organization to provide specialized medical care, indicating the diagnosis of the disease (state), the diagnosis code for the ICD-10, a medical organization, which is recommended to send a patient.
20. Extract from the Protocol of the Commission of a Medical Organization, providing high-tech medical care, within five working days (no later than the term of the planned hospitalization) is sent by means of a specialized information system in a guide for a medical organization and (or) OUN It is issued to the patient (his legal representative) on a written application or is sent to the patient (its legal representative) by means of postal and (or) electronic communications.
21. According to the results of the provision of high-tech medical care, medical organizations provide recommendations for further observation and (or) treatment and medical rehabilitation with the design of relevant entries in the patient's medical records.
22. The direction of patients from the citizens of the Russian Federation, whose health care provision in accordance with the legislation of the Russian Federation refers to the maintenance of the Federal Medical and Biological Agency (hereinafter referred to as FMBA of Russia), in the subordinate FMBA of Russia Federal Medical Organizations to provide high-tech medical care is carried out by FMBA Russia.
23. What is VMM?
High-tech medical care is assistance provided to the patient with the application of new technologies in medicine. The VMM includes therapeutic and diagnostic procedures, the range of services is in specialized medical centers. At the present stage, this type of care is carried out in 22 directions of treatment and diagnostics. The list includes topical types of operational interventions, as well as otolaryngology, pediatrics, combstiology and other complex, difficult to cure traditional methods of the disease.
In Russia, the number of hospitals is constantly growing, where modern equipment for the treatment of new generation medical technologies is established. The Ministry of Health of the Russian Federation annually approves a list of medical organizations that can provide VMM, and its volumes.
Financing
Initially, high-tech medical care to the population was financed only at the expense of the federal budget. Since 2014, the situation has become changed and more services began to be financed by the OMS Foundations system. High-tech medical care is divided into two types.
The separation is determined by the VMP list included in the assistance of the OMS system, and the rest of the help that is paid from the federal budget. That is, for citizens of the VMP - free, but there are certain conditions and procedures according to which it can be obtained.

In 2017, 146 federal medical institutions are funded from the OMS funds, where citizens have a VMP in addition to the basic insurance program.

A full list of high-tech medical care contains a large number of types of services - from therapeutic surgical interventions to diagnostic operations. You can get acquainted with him in any medical institution, the list includes more than 200 names of the VMM.
The patient receiving high-tech assistance in a medical institution does not carry any costs in connection with the procedures carried out. All types of services, consumables, medicines, work of doctors and sisters are covered at the expense of OMS or federal funds. It is worth remembering that the diagnostic procedures carried out before receiving the appointment on the WPM are not included in the financing of specialized assistance programs.
After the patient was provided with the necessary medical high-tech assistance, he receives a list of recommendations for further actions. It may include rehabilitation events or further treatment. All recommendations are described in the patient's medical card.
Who can get VMM?
All citizens of the Russian Federation have the right to receive VMM. A document that makes it possible to apply to a specialized hospital is the direction of the attending physician where the patient's medical testimony is prescribed to conduct certain procedures. The attending physician also sends a patient for additional examinations in order to identify contraindications to the WPM. For help funded by federal budget programs, it takes a little longer.
Having collected the results of all studies, the specialist is preparing a package of documents for transfer to the commission engaged in the selection of patients for VMM. Help for OMS does not require a doctor to appeal to the commission, the package of documents is immediately sent to the center where there are appropriate equipment and personnel. Cases of refusal
High-tech medical care every institution is in a certain amount (by quotas). If all the volumes of the necessary assistance are exhausted in a particular institution, the patient is entered into a list of waiting, which can be tracked in online mode on the website of the Ministry of Health.
Sometimes the Federal Medical Center, having considered the patient's problem, the diagnosis and research results, gives a refusal to conduct a VMM. The refusal can be motivated by the fact that the effect of the alleged treatment will be completely opposite and will lead to complications that require great efforts to maintain health than before the intervention of the VMP. If the patient does not agree with the refusal to assist him, it may apply to a complaint with the health authorities - local or territorial supervision services (Roszdravnadzor).
Emergency Cases and VSM

Preparation
Often patients are interested in such a type of operations as the replacement of eye lens. Where do VMP?
Since 2015, a procedure for which high-tech medical care is provided to the patient (order of the Ministry of Health of the Russian Federation dated December 29, 2014 No. 930n). You can get VMM in almost all major cities of Russia. Now there are more than 200 specialized hospitals and centers for the provision of the necessary range of services.
The organization of the provision of high-tech medical care consists of several points:
- Medical institution. WPM is provided in a hospital with a round-the-clock observation of the patient or in day hospital, if there is no need for constant observation of the patient's condition.
- Financing of the VMM program.The provision of VPM types is carried out in accordance with the approved list. It includes assistance types provided under the Basic OMS program, a list of VMM, carried out at the expense of the federal budget.
- Patient direction.The hospitalization of the patient occurs with filing and signed by the attending physician, as well as the head of the medical institution on the branded letterhead of the guide organization. Document package is attached to the direction: extract from the medical card with the diagnosis, the code of the disease, the results of studies confirming the diagnosis; Copies of documents: passport or birth certificate, medical insurance policy.
- Logistics. A complete set of necessary documents within three days is sent to the organization, where specialized high-tech medical care on the OMS system or to the executive body of the subject of the Russian Federation is provided. The distribution occurs in accordance with the list, which indicates who finances this type of care.

Not all types of high-tech medical care are within the framework of the OMS system. In the second section of the decrees, a list of diseases funded from the federal budget from the FOMS, other intergovernmental transfers are reflected. How to patient to issue a coupon for the provision of VMP
The decision on the need for treatment is accepted in the polyclinic hospital, to which the patient applies to the place of registration. The final decision on issuing a CTMM card or not, is accepted at the regional level for 10 days.

List of documents for talon VMM
List of documents for the provision of high-tech medical care for all persons:
- the conclusion of the Chief Freelance Specialist of the Ministry of Health by the Profile of the Disease, which states that a person needs to be provided. In conclusion they write a type of care, the name of the federal center, where a person will undergo treatment;
- extract from the outpatient map of the patient in print with the description of the survey results;
- direction indicating the diagnosis code on the ICD-10;

All adult patients (from 18 years) will need:
- passport of a citizen of the Russian Federation;
- polis Oms;

Children up to 18 will need:

What clinics are assisted on the coupons of VMP
- High-tech medical care included in the basic health insurance program is provided by organizations included in the registry.
- Help not included in the basic OMS program provide:
- FGU of the Ministry of Health of the Russian Federation in accordance with the list (part 6 of Art. 34 of Law No. 323-FZ; paragraph 1 of the order of the Ministry of Health of Russia of December 28, 2015 No. 1014n);
- medical organizations, the list of which is approved by the authorized body of the executive authority of the subject of the Russian Federation (Health Management, OUS) (Part 7 of Art. 34 of Law No. 323-FZ; p. 7, 8 of the Procedure).
The medical organization in which the patient is sent must be connected to the program assistance program, have a license and correspond to a number of criteria. At the same time, annually requires an application for participation in a strictly allotted time.
- How to become a member of the basic program of the OMS
Organizations wishing to provide high-tech assistance next year are obliged to send notice until September 1 of the current year in the territorial Fund. The territorial fund checks documents and medical organization for compliance with the criteria. The representative of the clinic has the right to be present when checking. Members of the Expert Council, which included specialists of the Ministry of Health, FAS, Roszdravnadzor, FFOMs, FMBA, Fano and institutions, analyze the paper and remove the verdict. In which volume is allocated funds for TALM TALON TALON
The chairman of the government reviews annually and approves a list of diseases and deviations, species and treatment methods. The ruling indicates the statutors of financial costs per unit volume (the cost of the procedure performed), as well as the number of allocations. Most of the types of high-tech assistance are rated within 50-200 thousand rubles.
At the provision of VPM outside OMS in the budget for 2019, 100.8 billion rubles were laid:
- 94.6 billion rubles. - for federal medical institutions;
- 6.4 billion rubles. - Dotations for regions.
For the treatment of many diseases, patients lack allocated tools, so they can only calculate the quota, and not to cover all expenses. If the cost of the service exceeds the permissible limit, the patient takes part of the material costs for itself or refuses to help.
How to finance the clinics
At high-tech assistance on the OMS, the means are allocated to the territorial funds in the form of subventions, and on VMM not under the state program are charged from federal institutions directly. The regional budget participates in financing in rare cases.
The Regional Commission distributes the amount of assistance between clinics. Commission members take into account the standards for WMP, the volume of consumption of medical care in the institution, the number of insured persons attached to the hospital, etc. The treatment of patients is allocated for the amount of the law and reflected in the ruling (from 50 to 200 thousand rubles in 2019).
Important! Private clinics, which can also provide VMM from February 13, 2019, receive subsidies only if they are licensed, included in the registry. The procedure for granting subsidies was approved in the Government Decision No. 56 of January 29, 2019.
What organizations can spend funds
- Basic OMS program
Medorganization receives the amount that you can spend on the purchase of software, databases, communication, fire fighting, protection, instrument check, etc. That is, the medical organization has the right to spend the funds accrued for assisting on the coupons of the WPM, on general economic needs.
- Assistance on the coupons of VMP outside OMS
The cost standard includes not only generality needs, but also funds for assistance, including the salary of doctors, medicines, the purchase of movable property, etc.
Regardless of what basis, a medical organization provides high-tech assistance, it is obliged to report on the treatment, expenditure of funds. If defects are detected, the institution deprives licenses, and later its leadership cannot submit an application for participation in the program.
