1. To approve the attached procedure for organizing high-tech medical care using a specialized information system.

2. The procedure for organizing the provision of high-tech medical care using a specialized information system

I. Organization of high-tech medical care

1. This procedure establishes the rules for organizing high-tech medical care using a specialized information system in medical organizations providing high-tech medical care, unless otherwise provided by international treaties of the Russian Federation.
Order of the Ministry of Health of Russia of May 29, 2015 N 280N.

2. High-tech medical care, which is part of specialized medical care, includes the use of new complex and (or) unique methods of treatment, as well as resource-intensive treatment methods with scientifically proven efficacy, including cellular technologies, robotic equipment, information technologies and gene methods Engineering developed on the basis of the achievements of medical science and related industries of science and technology.

3. High-tech medical care is provided in accordance with the procedures for providing medical care and based on medical care standards.

4. High-tech medical care is in the following conditions:

4.1. In day hospital (under conditions providing for medical observation and treatment during daytime, but not requiring round-the-clock medical observation and treatment);

4.2. Stationary (in conditions providing round-the-clock medical observation and treatment).

5. High-tech medical care is provided in accordance with the list of high-tech medical care species, established by the program of state guarantees of free provision of medical care citizens, which includes:

5.1. A list of high-tech medical care types included in the basic compulsory medical insurance program, the financial support of which is carried out at the expense of subventions from the budget of the Federal Fund for Compulsory Medical Insurance Bids of Territorial Mandatory Medical Insurance Funds;

5.2. The list of high-tech medical care types not included in the basic compulsory health insurance program, the financial support of which is carried out at the expense of the funds provided by the federal budget from the budget of the federal funds for compulsory medical insurance in the form of other intergovernmental transfers in accordance with the Federal Law on the Budget of the Federal Fund for Mandatory Medical Insurance For the next fiscal year and on the planning period.

6. High-tech medical care for the list of species included in the basic compulsory health insurance program is provided by medical organizations included in the register of medical organizations operating in the field of compulsory health insurance.

7. High-tech medical care for the list of species not included in the basic health insurance program is:

7.1. Federal government agencies, the list of which is approved by the Ministry of Health of the Russian Federation in accordance with Part 6 of Article 34 of the Federal Law of November 21, 2011 N 323-FZ "On the Fundamentals for the Health of Citizens in the Russian Federation";

7.2. Medical organizations, a list of which is approved by the authorized body of the executive authority of the constituent entity of the Russian Federation (hereinafter referred to as a list of medical organizations) in accordance with Part 7 of Article 34 of the Federal Law of November 21, 2011 N 323-FZ "On the Fundamentals of the Health of Citizens in the Russian Federation".

8. The authorized executive authority of the constituent entity of the Russian Federation submits a list of medical organizations in the Ministry of Health of the Russian Federation for December 20, preceding the reporting.

9. The federal fund of compulsory health insurance on the basis of information of territorial compulsory medical insurance funds is a list of medical organizations included in the register of medical organizations operating in the field of compulsory health insurance and providing high-tech medical care providing high-tech medical care. Included in the basic compulsory medical insurance program, up to December 10 of the year preceding the reporting.

In the event of amendments to the list of medical organizations included in the register of medical organizations operating in the field of compulsory medical insurance and providing high-tech medical care, the federal fund of compulsory medical insurance submits to the Ministry of Health of the Russian Federation information about the change in the specified list within 30 calendar days from Dates change.

10. The Ministry of Health of the Russian Federation forms a list of medical organizations in a specialized information system providing high-tech medical care in accordance with paragraphs 7-9 of this Procedure, up to December 30 of the year preceding the reporting.

II. Direction for high-tech medical care

11. Medical indications for the provision of high-tech medical care determines the attending physician of a medical organization in which the patient undergoes diagnosis and treatment in the provision of primary specialized health care and (or) specialized medical care, taking into account the right to choose a medical organization.

The presence of medical testimony to provide high-tech medical care is confirmed by the decision of the medical commission of this medical organization, which is issued by the Protocol and is submitted to the patient's medical documentation.

12. Medical indications for the provision of high-tech medical care is the presence of a patient with a disease and (or) states requiring the use of high-tech medical care in accordance with the list of types of high-tech medical care.

13. In the presence of medical testimony to provide high-tech medical care, confirmed in accordance with paragraph 11 of this Procedure, the attending physician medical organization in which the patient undergoes diagnosis and treatment in the provision of primary specialized health care and (or) specialized medical care ( Next - Guide Medical Organization) issues a direction for hospitalization to provide high-tech medical care on the letterhead of a guiding medical organization, which should be written on hand or in print, certified by the personal signature of the attending physician, personal signature of the head of medical organization (authorized person), seal Guide medical organization and contain the following information:

13.1. Surname, first name, patronymic (if any) patient, date of birth, registration address at the place of residence (stay);

13.2. The number of compulsory health insurance and the name of the insurance medical organization (if available);

13.3. Insurance certificate of compulsory pension insurance (if available);

13.4. The diagnosis code of the main disease in the ICD-10;

13.5. Profile, name of high-tech medical care in accordance with the list of types of high-tech medical care shown by the patient;

13.6. The name of the medical organization in which the patient is sent to provide high-tech medical care;

13.7. Surname, name, patronymic (if any) and the position of the attending physician, contact phone (if available), email address (if available).

14. The following patient documents are attached to the hospitalization for high-tech medical care:

14.1. Extract from medical records certified by the personal signature of the attending physician, personal signature of the head (authorized person) of a guiding medical organization containing a diagnosis of the disease (state), the diagnosis code for the ICD-10, information on the patient's health status, the results of laboratory, instrumental and other types of research confirming the diagnosed diagnosis and the need to provide high-tech medical care.

14.2. Copies of the following patient documents:

a) the document certifying the patient's identity (the main document certifying the identity of a citizen of the Russian Federation in the Russian Federation is a passport;

c) the policy of compulsory medical insurance of the patient (if available);

d) insurance certificate of compulsory pension insurance (if available);

14.3. Consent to the processing of personal data of the patient and (or) of his legal representative.

15. Head of the Directory Medical Organization or other authorized by the head of a medical organization is a set of documents provided for in paragraphs 13 and 14 of this Procedure, within three working days, including through a specialized information system, postal and (or) electronic communications:

15.1. The medical organization included in the register of medical organizations operating in the field of compulsory health insurance, in the event of a high-tech medical care included in the basic health insurance program (hereinafter - the host medical organization);

15.2. The executive authority of the constituent entity of the Russian Federation in the field of health (hereinafter - the OUS) in the event of a high-tech medical care, not included in the basic compulsory medical insurance program.

16. The patient (his legal representative) has the right to independently submit a decorated set of documents in the UUZ (in the case of high-tech medical care, not included in the basic health insurance program) or to a host medical organization (in the case of a high-tech medical care included in the basic program compulsory health insurance).

17. In the direction of the patient in the receiving medical organization, the registration of high-tech medical care on the patient for the provision of high-tech medical care (hereinafter referred to as the provision of VMM) with the use of a specialized information system provides a host medical organization with the attachment of a set of documents provided for in paragraphs 13 and 14 of this Procedure.

18. In the direction of the patient for the provision of high-tech medical care, not included in the basic program of compulsory medical insurance, the execution of the Talon for the provision of a VMM with the use of a specialized information system provides an OUC with the attachment of the set of documents provided for by 13 and 14 of this Procedure and the conclusion of the commission of the executive authority of the subject The Russian Federation in the field of health care on the selection of patients to provide high-tech medical care (hereinafter referred to as the OUN Commission).

18.1. The term for the preparation of the decision of the Commission for the confirmation of the presence (on the absence) of medical testimony to send a patient to the receiving medical organization to provide high-tech medical care should not exceed ten working days from the date of receipt of a set of documents provided for in paragraphs 13 and 14 of this Procedure.

18.2. The decision of the OUZ Commission is issued by the Protocol containing the following information:

18.2.1. The basis of the creation of the OUZ Commission (details of the regulatory legal act);

18.2.2. The composition of the Commission of the OUS;

18.2.3. Patient data in accordance with the identity document (surname, name, patronymic, date of birth, data on the place of residence, (stay));

18.2.4. Diagnosis of the disease (state);

18.2.5. Conclusion of the OUZ Commission containing the following information:

a) on confirmation of medical testimony to send a patient to a medical organization for the provision of high-tech medical care, a diagnosis of the disease (state), the diagnosis code for the ICD-10, the code of the type of high-tech medical care in accordance with the list of types of high-tech medical care, the name of the medical organization in which the patient is sent to the provision of high-tech medical care;

b) on the lack of medical indications for the direction of the patient to the medical organization to provide high-tech medical care and recommendations for further medical supervision and (or) treatment of the patient in the profile of its disease;

ICD-10, the name of the medical organization, which is recommended to send a patient for an additional examination.

18.3. The OUZ Commission's decision is made in two copies, one copy is subject to storage for 10 years in the UUC.

18.4. An extract from the protocol of the decision of the Uous Commission is sent to the Guide Medical Organization, including by postal and (or) electronic communications, and is also issued to the patient (his legal representative) on a written statement or is sent to the patient (his legal representative) by means of postal and ( or) electronic communications.

19. The basis for the hospitalization of the patient to the host medical organization and medical organizations provided for in paragraph 7 of this Procedure (hereinafter referred to as medical organizations providing high-tech medical care) is the decision of the medical institution of a medical organization in which the patient is sent to the fear of patients to provide high-tech Medical care (hereinafter referred to as the Commission of a Medical Organization, providing high-tech medical care).

19.1. The Commission of a Medical Organization, providing high-tech medical care, is formed by the head of a medical organization that provides high-tech medical care.

19.1.2. The Regulation on the Commission of a Medical Organization, providing high-tech medical care, its composition and procedure are approved by the Order of the Head of Medical Organization, which provides high-tech medical care.

19.2. The Commission of a Medical Organization, which provides high-tech medical care, makes a decision on presence (on the absence) of medical testimony or presence of medical contraindications to hospitalize the patient, taking into account the types of high-tech medical care, not exceeding seven working days from the date of registration on the patient of the coupon on The provision of VMM (except for the emergence of emergency, including emergency specialized medical care).

19.3. The decision of the Commission of a Medical Organization, which provides high-tech medical care is issued by the protocol containing the following information:

1) the basis of the creation of a medical organization commission providing high-tech medical care (details of the order of the head of the medical organization providing high-tech medical care);

2) the composition of the Commission of a Medical Organization providing high-tech medical care;

3) patient data in accordance with the identity document (surname, name, patronymic, date of birth, data on the place of residence (stay));

4) Diagnosis of the disease (state);

5) Conclusion of the Commission of a Medical Organization providing high-tech medical care containing the following information:

a) about the presence of medical testimony and the planned date of the patient's hospitalization into a medical organization providing high-tech medical care, a diagnosis of the disease (state), the diagnosis code for the ICD-10, the code of the type of high-tech medical care in accordance with the list of types of high-tech medical care;

b) on the absence of medical testimony to hospitalize the patient to a medical organization providing high-tech medical care with recommendations for further medical supervision and (or) treatment of the patient on the profile of its disease;

c) on the need for an additional examination (indicating the necessary amount of additional examination), diagnosis of the disease (state), the diagnosis code for the ICD-10, indicating a medical organization, which is recommended to send a patient for an additional examination;

d) on the presence of medical testimony to direct the patient to the medical organization to provide specialized medical care, indicating the diagnosis of the disease (state), the diagnosis code on the ICD-10, a medical organization, which is recommended to send the patient;

e) on the presence of medical contraindications to hospitalize the patient to a medical organization providing high-tech medical care, indicating the diagnosis of the disease (state), the diagnosis code for the ICD-10, recommendations for further medical examination, observation and (or) treatment of the patient on the disease profile ( status).

20. Extract from the Protocol of the Commission of a Medical Organization, providing high-tech medical care, within five working days (no later than the term of the planned hospitalization) is sent by means of a specialized information system, postal and (or) electronic communications in a guide medical organization and (or) OUZ, which I made a coupon for the provision of VMM, and is also issued to the patient (his legal representative) on a written statement or sent to the patient (his legal representative) by means of postal and (or) electronic communications.

21. According to the results of the provision of high-tech medical care, medical organizations provide recommendations for further observation and (or) treatment and medical rehabilitation with the design of relevant entries in the patient's medical records.

22. The direction of patients from the citizens of the Russian Federation, whose health care provision in accordance with the legislation of the Russian Federation refers to the maintenance of the Federal Medical and Biological Agency (hereinafter referred to Russia.

23. The direction of patients from the number of military personnel and persons equated in medical support to military personnel, medical organizations providing high-tech medical care, is carried out in accordance with Article 25 of the Federal Law of November 21, 2011 N 323-FZ "On the basis of the health of citizens in Russian Federation" .

24. Starting 01.01.2015 The procedure for the provision of high-tech medical care was established by the Order of the Ministry of Health of the Russian Federation of December 29, 2014 N 930N "On approval of the procedure for organizing the provision of high-tech medical care with the use of a specialized information system" (hereinafter referred to as N 930N).

In day hospital (under conditions providing for medical observation and treatment during daytime, but not requiring round-the-clock medical observation and treatment);

High-tech medical care is provided in accordance with the list of high-tech medical care species established by the Government Program. This list includes:

In the medical organization included in the register of medical organizations operating in the field of OMS, in the event of a high-tech medical care included in the basic program of the OMS;

The executive authority of the subject of the Russian Federation in the field of health care (hereinafter - the OUS) in the event of a high-tech medical care, not included in the Basic CMF program.

Note. The decision of the OUZ Commission is issued by the Protocol, which is drawn up in two copies, one of which is subject to storage in the UUZ for 10 years.
