What is included in the survey

People who are 39 years old at the time of the prophylactic examination are prescribed additional studies. Their list also depends on gender:

  • Ultrasound of the abdominal cavity and small pelvis is performed every 6 years.
  • An ultrasound of the mammary glands for women is planned every three years until the age of 50, then - after a year.
  • Glaucoma is diagnosed by measuring eye pressure.
  • From the age of 45, the risk of developing colon cancer increases, so a fecal occult blood test is performed.
  • From the age of 51, the patient is sent for examination to a neurologist, and men donate blood for the detection of an antigen indicating prostate cancer.

The goal of the program is to identify signs of chronic non-infectious diseases, to diagnose the development of oncology. Based on the results of the first stage of the examination, the therapist gives a referral for tests or consultations of narrow specialists. A medical passport of the patient is created, in which all information about the state of his health is entered. After all consultations and analyzes, the therapist assigns the ward one of three health groups, on the basis of which procedures, exercise therapy or treatment are prescribed.

Medical examination of pensioners

In the program of medical examination of the population, there is no separate article regulating the examination of pensioners. This category can undergo free medical examination in a polyclinic on a general basis. The groups of citizens who can undergo a medical examination annually, regardless of age, have been identified:

  • disabled participants of hostilities, World War II;
  • WWII veterans who were disabled as a result of hostilities, general illness or injury;

Where and when can I undergo medical examination?

Citizens undergo clinical examination in a medical organization in which they receive primary health care: in a polyclinic, in the center (department) of general medical practice (family medicine), in a medical outpatient clinic, a medical unit.

In addition, there are special rules. The law defines scheduled examinations, which must be carried out monthly for up to 1 year, every 3 months - up to 2 years, after six months - up to 3 years. The difference lies in the more in-depth nature of screening when children reach the age of 1, 3, 6, 7, 10, 14, 15, 16 and 17 years old. Features of the organization

In the new modern structure of the organization of medical examination, there are the following features:

  1. A citizen of the Russian Federation who has a medical insurance policy has the right to undergo a free examination.

The clinical examination begins in the office of the district therapist - what does it include? It will take at least two visits to the clinic to pass the first stage of the examination. For the first visit, you need 2 to 6 hours of free time. Clinical examination is carried out in 2 stages.

The first stage of clinical examination (screening) is carried out in order to identify signs of chronic non-infectious diseases in citizens, risk factors for their development.

The first stage includes:

1) a survey (questionnaire) aimed at identifying chronic non-infectious diseases, risk factors for their development, the consumption of narcotic drugs and psychotropic substances without a doctor's prescription;

7) electrocardiography at rest (for men over the age of 35, for women aged 45 and over, and for men under the age of 35 and women under the age of 45 - during the initial medical examination);
8) examination by a paramedic (midwife), including taking a smear from the cervix for cytological examination) (for women up to 69 years old inclusive);
9) fluorography of the lungs;

12) a detailed clinical blood test (for citizens aged 39 years and older with a frequency of 1 time in 6 years instead of a clinical blood test);
13) a biochemical general therapeutic blood test (in the amount of at least determining the level of creatinine, total bilirubin, aspartate aminotransaminase, alanine aminotransaminase, glucose, cholesterol - for citizens aged 39 years and older with a frequency of 1 time in 6 years instead of the studies provided for in paragraphs 4 and 5);
14) general urine analysis;
15) study of feces for occult blood (for citizens aged 48 to 75 years);
16) ultrasound examination (ultrasound) of the abdominal cavity and pelvic organs (for citizens aged 39 years and older with a frequency of once every 6 years), and for men who have ever smoked in life, also ultrasound of the abdominal aorta at the age of 69 or 75 years old;
17) measurement of intraocular pressure (for citizens aged 39 and older);
18) reception (examination) of a general practitioner, including establishing a diagnosis, determining a group of health status, a group of dispensary observation, conducting brief preventive counseling, including recommendations on healthy eating, level of physical activity, cessation of tobacco smoking and harmful alcohol consumption, determination of medical indications for examinations and consultations as part of the second stage of clinical examination.

The second stage includes:

1) duplex scanning of the brachycephalic arteries (according to indications);
2) esophagogastroduodenoscopy (according to indications);
3) examination (consultation) by a neurologist (according to indications);
4) examination (consultation) by a surgeon or urologist (for men aged 42 to 69 years according to indications, as well as for men regardless of age in case of suspicion of prostate cancer based on ultrasound results);
5) examination (consultation) by a surgeon or a coloproctologist (with a positive fecal occult blood test), for citizens aged 45 years and older - if other medical indications are detected;
6) colonoscopy or sigmoidoscopy (as prescribed by a surgeon or coloproctologist);
7) determination of the lipid spectrum of blood (for citizens with an identified increase in the level of total cholesterol in the blood);
8) spirometry (for citizens with suspected chronic broncho-pulmonary disease based on the results of a questionnaire survey, smokers and on the referral of a general practitioner);
9) examination (consultation) by an obstetrician-gynecologist (for women with identified pathological changes based on the results of a cytological examination of a smear from the cervix and (or) mammography, ultrasound of the uterus and ovaries);
10) determination of the concentration of glycated hemoglobin in the blood or a glucose tolerance test (for citizens with an identified increase in blood glucose levels);
11) examination (consultation) by an otorhinolaryngologist (for citizens aged 75 years and older in the presence of medical indications based on the results of a questionnaire survey or examination by a general practitioner);
12) a blood test for the level of prostate-specific antigen (as prescribed by a surgeon or urologist for men with suspected prostate cancer based on the results of a survey, examination, digital examination or ultrasound of the prostate gland);
13) examination (consultation) by an ophthalmologist (for citizens aged 39 years and older who have increased intraocular pressure, and for citizens aged 75 years and older who have decreased visual acuity that cannot be corrected by spectacles, revealed by the results of a questionnaire survey);
14) individual in-depth preventive counseling or group preventive counseling (patient's school) in the department (office) of medical prevention (health center, medical assistant's health center or medical assistant-obstetric point) (for citizens with identified risk factors for the development of chronic non-communicable diseases who have these diseases or have high and very high absolute total cardiovascular risk);
15) reception (examination) of a general practitioner, including the establishment (specification) of the diagnosis, determination (specification) of the health status group, determination of the dispensary observation group (taking into account the conclusions of medical specialists), as well as referral of citizens in the presence of medical indications for additional examination, not included in the scope of medical examination, to receive specialized, including high-tech, medical care, for spa treatment.

If a citizen is identified in the process of clinical examination of medical indications for conducting research, examinations and activities that are not included in the scope of medical examination, they are assigned and performed to the citizen, taking into account the provisions of the procedures for the provision of medical care according to the profile of the identified or suspected disease (condition) or standards of medical care approved in accordance with part 2 of article 37 of the Federal Law of November 21, 2011 No. 323-FZ "On the basics of protecting the health of citizens in the Russian Federation."

How to prepare for medical examination? *

  • On the first visit, it is advisable to come to the medical organization in the morning, on an empty stomach, before performing any physical activity, including morning exercises.
  • Take with you a morning portion of urine in the amount of 100-150 ml.
  • If it is necessary to conduct a study of feces for occult blood, check with the clinic which method is used for the analysis. If immunochemical, then no dietary restrictions are required. If the study is carried out by a different method, then in order for the test result to be correct, within 3 days before the examination, give up food with a high content of iron (meat, apples, white beans), laxatives and enemas, iron preparations, aspirin and ascorbic acid (in the absence of contraindications).
  • If you underwent medical examinations in the current or previous year, take documents confirming this and show them to medical workers before starting a medical examination.
  • The scope of preparation for the second stage of clinical examination will be explained to you by the local doctor.
Preventive medical examination of the adult population

The procedure for conducting a preventive medical examination was approved by Order of the Ministry of Health of the Russian Federation dated December 6, 2012 No. 1011n.

Who can get through?

A preventive medical examination is carried out in relation to citizens aged 18 and over.

For what purpose is it carried out?

A preventive medical examination allows you to:

  • to identify risk factors and early signs of cardiovascular, oncological, bronchopulmonary diseases and diabetes mellitus;
  • receive timely assistance in correcting risk factors and identified deviations in health.

Where and how is it carried out?

A preventive medical examination is free of charge. It can be passed in a polyclinic, in which a citizen receives primary health care, once every two years.

On the day of contacting the clinic for a preventive examination, you must have a passport and a policy of compulsory medical insurance with you.

In the year of passing the prophylactic medical examination, a preventive medical examination is not carried out.

What examinations and doctors' consultations are included in a preventive examination?
A preventive medical examination includes:

1. Survey (questionnaire) in order to identify chronic non-infectious diseases, risk factors for their development, consumption of narcotic drugs and psychotropic substances without a doctor's prescription;
2. Anthropometry (measurement of standing height, body weight, waist circumference), calculation of body mass index;
3. Measurement of blood pressure;
4. Clinical blood test;
5. Determination of the level of total cholesterol in the blood;
6. Determination of blood glucose levels;
7. Reception of a general practitioner, including prophylactic counseling on the correction of identified risk factors and, if there are medical indications, referral of citizens to receive specialized, including high-tech, medical care for spa treatment.

For women aged 39 and over, additionally:

8. Mammography.
For citizens aged 45 and over, additionally:
9. Study of feces for occult blood.
For citizens under the age of 65, additionally:
10. Determination of the total cardiovascular risk.

Preventive medical examinations of children

Preventive medical examinations of children are carried out free of charge, within the framework of the program of state guarantees of free provision of medical care to citizens and territorial programs of state guarantees of free provision of medical care to citizens, including within the framework of the territorial program of compulsory medical insurance.

Who can get through?

Minors between the ages of 0 and 17 at certain age periods.

For what purpose are they carried out?

Preventive medical examinations of minors are carried out at certain age periods in order to timely identify pathological conditions, diseases and risk factors for their development, as well as in order to form groups of health status and develop recommendations for minors.

Where and how can you go?

A preventive examination is carried out in a medical organization in which the child receives primary health care.

The doctor responsible for conducting the preventive examination (pediatrician, district pediatrician, general practitioner (family doctor), no later than 5 working days before the start of the examination, must give (send) a referral to the minor (his legal representative) for a preventive examination with an indication of the list of examinations by specialist doctors and studies, as well as the date, time and place of their conduct.

On the day of contacting a medical organization in order to undergo a preventive examination, you must have a compulsory medical insurance policy and a referral for a preventive examination issued by the doctor responsible for its conduct.

What does the preventive examination program include?

The list of examinations by specialist doctors, laboratory, instrumental and other studies as part of a preventive examination.

When conducting preventive examinations, the results of examinations by specialist doctors and studies are taken into account, entered in the history of the child's development, the prescription of which does not exceed 3 months from the date of the examination and (or) research, and in children under the age of 2 years, the results of examinations and studies are taken into account , the prescription of which does not exceed 1 month from the date of inspection and (or) research.

A preventive examination is carried out in one stage and is completed in the case of examinations by specialist doctors and studies provided for by the examination program.

Preventive examination is carried out in two stages in case of suspicion of the presence of a disease (condition) in a child, the diagnosis of which cannot be established during examinations included in the examination program. In this case, the preventive examination is considered completed after additional consultations, research and (or) obtaining information about the child's health from other medical organizations.

What is the time frame for a preventive examination?

The total duration of the first stage of a preventive examination should be no more than 10 working days, the first and second stages - no more than 45 working days.

In addition to preventive examinations, preliminary and periodic medical examinations are carried out for children.

Preliminary medical examinations are carried out upon admission of the child to educational institutions in order to determine the student's compliance with the educational requirements.

Periodic medical examinations are carried out in order to dynamically monitor the state of health of students, to timely identify the initial forms of diseases, early signs of the impact of harmful and (or) dangerous factors of the educational process on their health and to identify medical contraindications to continuing their studies.

The procedure for conducting dispensary observation

1. Dispensary observation is a dynamic observation, including the necessary examination, of the state of health of persons suffering from chronic diseases, functional disorders, other conditions, in order to timely identify, prevent complications, exacerbation of diseases, other pathological conditions, their prevention and the implementation of medical rehabilitation of these persons.

2. Dispensary observation of citizens suffering from chronic non-communicable diseases, which are the main cause of disability and premature mortality of the population of the Russian Federation, as well as having the main risk factors for such diseases, is part of the complex of measures for conducting medical examination and preventive medical examinations of the population.

3. Dispensary observation is carried out by the following medical workers of a medical organization where a citizen receives primary health care:

    General practitioner, general practitioner (family doctor)

    Specialist doctors (according to the disease profile of a citizen)

    Physician at a medical prevention department or health center

    Feldsher of a feldsher-obstetric station in the event that the head of the medical organization assigns him certain functions of the attending physician

4. The medical professional referred to in clause 3:

    Keeps records of citizens under dispensary supervision

    Informs the citizen about the procedure, volume and frequency of dispensary observation

    Organizes and carries out dispensary appointments (examinations, consultations), examinations, preventive, therapeutic and rehabilitation measures

    If it is impossible for a citizen subject to dispensary observation to visit a medical organization due to the severity of the condition or impairment of motor functions, organizes a dispensary reception at home

5. The list of diseases (conditions), in the presence of which a group of dispensary observation by a general practitioner is established, including the recommended duration and frequency of dispensary observation - Appendix to the Order of 21.12.2012. N1344n “On approval of the procedure for conducting dispensary observation”. This is a table with diseases and frequency of doctor visits.

6. Dispensary reception by a medical worker includes:

    Assessment of the state of the citizen, collection of complaints and anamnesis, physical examination

    Appointment and assessment of laboratory and instrumental studies

    Establishing or clarifying the diagnosis of the disease

    Conducting a brief preventive counseling

    Prescribing preventive, therapeutic and rehabilitation measures for medical reasons, including referring a citizen to a medical organization providing specialized (high-tech) medical care, to a spa treatment, to a medical prevention department or a health center for in-depth individual preventive counseling and (or ) group preventive counseling (patient school)

    Explaining to a citizen with a high risk of developing a life-threatening disease or its complications, as well as to persons living with him, the rules of action for their development and the need to call an ambulance in a timely manner.
