The mechanism of action of tablets, the active substance of which is levonorgestrel (names of birth control pills after the act of Postinor, Escapel, Eskinor F) consists of three points:

  • Delay or block ovulation. Acting on the ovaries, levonorgestrel prevents the egg from escaping from the main follicle (when taking the drug on the eve of ovulation), since it suppresses the effect of gonadotropic hormones (FSH and LH).
  • The composition of cervical mucus changes. Among other things, levonorgestrel increases the viscosity of the mucus of the cervical canal. It becomes thick and slows down the process of sperm penetration into the uterus.
  • Inadmissibility of implantation of a fertilized egg. 2 mechanisms prevent the introduction of an egg into the uterine mucosa. The first is that the structure of the endometrium changes, since levonorgestrel causes the reverse development of proliferative processes in the endometrium, which excludes the possibility of its transition to the secretory phase, so necessary for complete ovulation. Thus, postinor and other drugs containing levonorgestrel have a mild abortive effect. In addition, levonorgestrel affects the peristalsis of the fallopian tubes, their contractions decrease, which disrupts the delivery (in time) of the fertilized egg to the uterine cavity for implantation.

    Mifepristone is used less frequently than levonorgestrel preparations and because of a wider range of contraindications. They cannot be recommended for women with anemia, kidney and liver failure, porphyria, and other serious pathologies of internal organs and systems.

    Breastfeeding is a categorical contraindication to the appointment of mifepristone, but levonorgestrel for the purpose of emergency contraception in lactating women can be used. A necessary condition for this is its intake after feeding the baby.

    There are, of course, exceptions to this rule when you need to eat exactly at the indicated time after admission. For example, with diseases of the gastrointestinal tract or endocrinopathies.
